The Infant Feeding Maternal Discharge Survey (IFMDS) is now available for all CHAMPS National hospitals! The IFMDS is a new CHAMPS resource designed to help hospitals gather meaningful feedback directly from mothers about their birth experiences and hospital care. The survey is available in both English and Spanish.

If your hospital chooses to participate, you will distribute the survey directly to patients. CHAMPS will provide monthly reports summarizing responses, allowing you to easily track trends and identify areas for improvement over time.

Example Report – An example report that includes patient responses collected between July 2025 and December 2025. As patients at your hospital complete the survey, CHAMPS will collect and summarize your data into tables and graphs similar to this example. These reports are intended to help you track progress over time and support targeted, data-driven quality improvement efforts.
